Visitor COVID Screening

Note: If you answer “yes” to any screening questions, you are asked to refrain from attending the site.

If you answer “no” to the screening questions and your condition changes before accessing the site or while on site during the event, you are asked to refrain from attending the site.

Have you experienced any of the following symptoms outside of your norm and that are not related to other known causes or medical conditions within the last 5 days (two or more COVID-19 vaccinations) or 10 days (fewer than two COVID-19 vaccinations)?

  • Fever/chills
  • New or worsening cough
  • Shortness of breath or difficulty breathing
  • Sore throat
  • Runny or stuffy/congested nose
  • Decrease or loss of sense of taste or smell
  • Diarrhea, stomach pain, nausea or vomiting
  • Headache (that is unusual or long-lasting)
  • Extreme fatigue
  • Body or muscle aches/joint pain
  • Pink eye
  • Skin rash of unknown cause (MB additional symptom)

In the last 5 days (two or more COVID-19 vaccinations) or 10 days (fewer than two COVID-19 vaccinations), have you:

  • Tested positive for COVID (PCR or Rapid Antigen) and am not yet cleared.
  • Been told to isolate by public health or health care provider and have not yet been cleared.

Do any of the following apply?

You live with someone who is currently isolating because of a positive PCR or Rapid Antigen COVID-19 test.
You live with someone who is currently isolating because of COVID-19-like symptoms.
You live with someone who is isolating while waiting for COVID-19 test results.

Respond “No” if you have received your booster dose of the COVID-19 vaccine; or you have completed your isolation after testing positive for COVID-19 in the last 90 days and your result is documented; or your household member is isolating because of symptoms but has already tested negative on one PCR test or two Rapid Antigen tests 24-48 hours apart and they have had symptom improvement for 24 hours or 48 hours if GI symptoms were present (nausea, vomiting, diarrhea).

*Note: If selecting “no” due to meeting these conditions, but are living with someone with an identified COVID-19 risk, you are reminded to continue strong COVID-19 mitigation measures.

Note: If in the last 14 days, you or a member of your household entered Canada from another country, and you do not meet the Government of Canada quarantine exemption requirements, you are asked to refrain from attending the site.
